I have noticed that every now and then monsters will turn invulnerable randomly. (not due to scroll screwups or post 5 mins in level) It is pretty rare, but I have had it happen a few times now. Luckily, none of them have been near a chest so I have been able to just run past them. I am not sure what is wrong in these instances. Just keep an eye out for it.

The few times I've had reports of this happen, the monsters do not flag you with the red star anyways, so you can just ignore them completely.
