What exactly do Worldstone Fragments do? And how many are in each set and where can you find them?

So there are 5 worldstone fragments. They are:
Green (Drops from Andariel)
Yellow (Drops from Duriel)
Blue (Drops from Mephisto)
Red (Drops from Diablo)
Gold (Drops from Baal)
