What is the way to open Altar of Cthulu ?

What's can i to do ?

stand next to it and you get an icon above your head, when you have the icon you can cube r'yleh caches to open them

each dreamlands boss has a chance to drop their own puzzle box, opening a puzzle box gets you a key (4 keys total)
one of each key cubed together makes another key that has a charge that lets you teleport over the cathedral wall in dreamlands
go into the cathedral and kill the avatar of Cthulhu to get your cthulhu item, the altar is in the bottom left corner of the room
dont forget to open a tp at the entrance or you won't be able to get in again if you die

each puzzle box has its own hint:
"Have you had the dream again? A hidden place where 7 stars align." - opened at the orifice in act 2 before duriel
"His whispers will drain your sanity, corrupting even the holiest of beings." - opened at izual
"His web of madness spreads into even the darkest lairs." - opened at the sparkly chest in the arachnid lair in a3
"At the bottom of the ocean even light must die." - opened on the small island behind anya in the frozen river
you will get the icon above your head for all of these so you know when you can open them (just cube them)

Why won't it let me open "sanity" box? I killed Baal, and have a "!" over my head, but still can't open the box via right click or cube. :/

Because it opens at Izual, not baal... lol That is typed up above in Research's post.
