Poison smiter is best for cathulhu. That guide can be found here. I died 1-3 times vs my last cathulhu with my poison smiter.

The first time I killed him, I'd swoop in with my trangs necro and poison him every 10 seconds. That took a long time and I had a lot of deaths.
