Sorry to dig up this old post.
I had the same problem, and to get a better resolution I did this :
- Launch D2VidTst.exe (In Diablo II folder)
- Ignore test (faster)
- Chose 3dfx Glide: 3dfx Glide option (instead of Direct3D or DirectDraw(2D))
