Oh and we also had an Omega Baal drop. I didn't get the error code though.

This seems to happen when two portals spawn in the same game. On approaching one of them you'll get the error and a crash. The other will work. Unfortunately there's no way to tell them apart.

Correct. It only happens when 2 portals spawn in the same game. At this time there is no fix. Its a really really low chance of it happening though. If you approach 2 portals, bring in a mule to save your game, try 1, if it crashs, try the other one and it will work.
